Handheld speckle entoptic perimetry apparatus

Perimetry is the act of detecting scotomas (blind spots) in the visual field due to different diseases such as glaucoma, diabetic retinopathy, etc. Conventionally, perimetry is done with a big bowl in which lights are presented and the patient has to press a button when the light is seen (static and kinetic perimetry). Such tests take a lot of time and require enough roomspace. They are less practical for screening purposes. (Indirect) screening is done with tonometers (for example with the puff of air at the optometrist) or with an apparatus called frequency doubling perimetry, but this is still a relatively large apparatus and has a low resolution.

This invention is based on a different method: white noise entoptic perimetry. By presenting a dynamic noise field, the patient can observe his/her own (new) scotomas. This can be used not only for quick screening of visual field problems, but also for actually showing the patient something is wrong with the vision to improve therapy adherence (a problem with for example glaucoma, since the patient in the beginning is not really aware of the visual field defects).

The noise field is very ingeniously not created with a display (which could only cover a limited field of view), but with speckles. Speckles are created by random constructive and destructive interference of coherent light (for example from a cheap laser diode). Besides being able to cover a large field of view with speckles (because it does not originate from an imaging optical system), another beautiful aspect of speckles is that they are relatively refraction/accommodation independent. So even without glasses or correct ocular focussing, the speckles will still be clearly ‘imaged’ on the retina. This all in a very small handheld package, potentially useful for opticians, optometrists, ophthalmologists, and possibly even emergency room personal or general practitioners.
